Vinod,
Corrosion, stress cracking, etc can lead to failure of plate and potential results catastrophic failure.

There area many parameters affecting relieving flow which rather same as tube rupture in shell & tube heat exchanger. One of the major parameter is the maximum area which is normally twice the maximum area of a "tube" cross sectional area. Read more here
