I am looking for an effective cause-effect analysis tool for a distillation plant that can be used for future reference to previous process related troubleshooting findings.

I don't see the following tools as specific to process plants. Root cause analysis is generally applicable in troubleshooting plant problems.

A good book for troubleshooting is :Troubleshooting  Process operations by Norman  P Lieberman  to support your Why , why ,..Study .
