I have to simulate a flow/heat transfer loop. The problem is that, for the heat exchanger I have only initial rated pressure drop/flow values. Many of tubes have now been plugged along with other adverse changes. How can I put this actual heat exchanger in my simulations? What calculations/measurements I will be required to do on the heat exchanger?

Do you have the bundle information such as the tube layout and baffle data ?

Baffle dimensions and other data is available to us. Number of plugged tubes is also known. What we do not know is the extent of clogging on tube side------- circuit that is to be modeled. 

On shell side we have temp., dp and flow instruments.

First thing to do is to use the shell side instruments and measure the flow and inlet and outlet temperatures. From this information you can calculate the heat load.
