Process Guidelines For Conducting A Hazop Review




A few weeks back I was asked to prepare process engineering guidelines for various process engineering activities undertaken in the process engineering discipline in my company. This was a very challenging and interesting assignment and required preparing descriptive word documents which would be easy to understand and follow by young engineers.

There were several guiidelines I was required to prepare and which I did in the time slot given to me. The one that I really enjoyed preparing and also found the most interesting was "Process Guidelines for Conducting a HAZOP Review". I am attaching an abridged version of the guideline for the benefit of the young engineers of the forum who may have to attend a HAZOP review meeting for the first time in the very near future. Some portion of the guideline is omitted for reasons of confidentiality, which, however does not affect the continuity of the description.

Enjoy reading and don't forget to provide your comments. Veteran engineers can probably add a lot of value by giving useful suggestions.

This German web site has useful "process" HAZOP guidewords (also in English) as excel and word documents. It also has a document in German (the bottom one) that is entitled Handover Protocol, which has a very useful template for documenting the design basis of safety for process plant. Babelfish is useful for translating these!

The number of guidewords is extensive, but definitely assists process-type HAZOPs when appropriate.

May I have a question relating to HAZOP re-validation? It is stated in OSHA – 1910 that "At least every five (5) years after the completion of the initial process hazard analysis, the process hazard analysis shall be updated and revalidated by a team meeting the requirements in paragraph (e)(4) of this section, to assure that the process hazard analysis is consistent with the current process.”

 

For a Facilitator, it is also quoted: "The process hazard analysis shall be performed by a team with expertise in engineering and process operations, and the team shall include at least one employee who has experience and knowledge specific to the process being evaluated. Also, one member of the team must be knowledgeable in the specific process hazard analysis methodology being used."

Is it compulsory if team leader/chairman should be someone independent of the project? Our field is now in Operation Phase. I would recommend to ask the 3rd party to lead this HAZOP but I couldnt find any standard or recommended guideline for this requirement. Please help to advise.

No, it is not compulsory. Some companies prefer to have a 3rd party facilitator for the purpose of neutrality and impartial view of the entire HAZOP for the process plant or unit. If all concerned are agreeable you can have an in-house HAZOP facilitator who is well versed with the HAZOP procedure.
