Positive Equipment Isolation Options

From a process point of view, what determines the best option for Positive Isolation on Equipment from these options below or others
 
Closed Valve.
Closed Valve with Open Bleeder
Double Block and Bleed 
Block and Bleed with Line Break
Block, Bleed, and Spectacle Blind
Double Block and Bleed with Blind 
Double Block and Bleed with Line Break

While closing two valves and voiding the space between them provides very good isolation and an ability to confirm isolation, it is not considered "positive isolation" because it is not certain.  Only a blind or physical removal of piping (dropping a spool) should be considered "positive isolation".   The need for valving and bleeds will depend on how much of the plant you want to depressuring before swinging a blind or spool.
