The licensor's engineers prepares the design package that includes heat and material balances, PFD's, P&ID's, equipment datasheets, instrument datasheets, et cetera.
The engineers at the EPC contractor use those documents to prepare requisitions (purchase orders) for all equipment and instrumentation, material take off (piping, valving, insulation, structural steel, concrete, et cetera). In addition required new utility systems, waste water treating, flare system, et cetera, are designed by the EPC contractor.
And the EPC contractor builds the whole unit at operating companies site.
The Operating Company initiates the project. They determine what new process units are required at what capacity, contact licensors and select one of them. Or they hire a design company to do that work. For big projects with multiple new units they may hire a Managing Contractor (usually another EPC company) to manage the whole project and supervise the work of licensors and EPC contractor.
How exactly it is organized depends on the Operating Company. Big companies can do a lot themselves and usually don't need an MC.
An Open Art design is a design that does not require a licensor but can be done by any experienced design group.
