The interim recommendations stemming from the ongoing investigation by the Chemical Safety Board of the November 15, 2014, toxic chemical leak that killed four workers at the DuPont insecticide plant in La Porte, Texas have been released for public knowledge today.  The report can be downloaded at:

I strongly encourage all chemical engineers to download a copy and study it carefully.  The final report is pending and has yet to be reviewed and issued, but these interim results are very important and will probably form the core findings of the total investigation of this tragic event.
