Dear Waheed

Good day

The most obvious reason is lack of deficiency of Vanadium or Potassium nitrite in the system. Please check the analysis at immediate out let of absorber and stripper as well
Please note that obtaining samples at the downstream of activated carbon filters will definitely give wrong results due to reduction properties of activated carbon used for de-coloration

As per UOPL recommendation, total vanadium should not be less than 0.7 wt percent and V5 should not be less than 0.4 wt percent.

This is a very serious issue and need to address immediately as it has resulted in equipment failure in past. Please, also do consult UOPL

Best regards
