My questions is: what are the procedures/code/standard should be followed for heat exchanger tubes plugging?

I know of no standards or guidelines for plugging heat exchanger tubes.  But I've had to plug a lot of them in the past.

I did what everyone else I know does:  get your maintenance department to machine a stock of steel (or the appropriate alloy) tapered plugs.  The angle of the taper is up to you and the machinist.  I left this detail up to the experienced machinist and the plugs worked well.
