When I was modeling my process in dynamic mode(my model is a little bit large, which is a section of plant), I got a problem.

I can run it in dynamic mode, everytime it goes well at the begining, but after 13.5mins, I got a error message said “Fluid Package Transition owned by 12-bb failed. select OK to view the object.”(12-bb is a stream comes from my absorber column). After I select OK, it shows me the dynamic page of my absorber then it stop (integrator holding). I've checked my fluid package I used,  I do only use UNIQUAC in my whole system. How can Fluid Package Transition happen?

Is anyone can help me with this, what is the reason?

And one more thing, why it show me this:

"Very low efficiency flash failed. Increasing efficiencies to 5% did not help." What's wrong with this?
