Salapao,

I believe you won't get much answer unless you supply your own work for us to review .

Basically we don't know what you are monitoring and which data are relevant for your plant , Did you set some KPIs ?

This kind of document should be easy to prepare with data collected on a daily basis or monthly basis .

If you use your favorite search engine and Youtube you will find some information on how to build a spreadsheet .

Dear Salapao ,

For this you may approach to your sr. officers to oversee how is the existing format.

But if I would have been in your place, I would have prepare Utility performance monitoring system as below :

1) Tabulating - Sr. No., Utility Name, Type of Fluid, Service, Phase, Design Ranges & parameters contributing Performance

( Such as Temperature , Pressure, Ph, Hydrocarbon Content -For Flare Llines, SO2 /CO2 content for Flare etc.)

2)  Tabulating - Operating values ( such as Temp. , pressure etc.)

We can make final remark column - OK / Not OK & also corrective measures, if any.

Comments / suggestions, if any , are most welcome.

Regards,

SPC
