Process Isolation Article - Progressive Improvement Scenario

Good morning all,

I am trying to locate an article which I have previously seen on risks associated with preparation of process isolations (possibly by the IChemE or AIChE).

The article in question presented a fictions example of process isolations being implemented prior to breaking containment. Risks associated with the initial proposal are identified and step-by-step incremental improvements are made (acknowledging any new risks introduced) until by the 7th or so iteration an acceptable (for the scenario) arrangement is arrived at.

Changes include (I think) things like:

- local venting of bleed valve introduces potential for build-up of flammable material
- bleed is routed to flare to remedy issue
- back pressure from flare causes issue with pressure built-up behind main isolation valve.

And so on.

I did not save the article at the time of reading so-as to avoid cluttering up my computer… in the timeless words for Mr Gump ‘I am not a smart (wo)man’.

Appreciate that this is not much to go on… however if any of the above rings any bells I would be thankful if you could point me in the right direction such that I can a copy of the article, as I should have during the first read.

I should add that the article was a standalone PDF - not part of a blog / forum / or otherwise.
