What you need is a copy of this book:

"Guidelines for Hazard Evaluation Procedures", 2nd Edition, Center for Chemical Process Safety, American Institute of Chemical Engineers

You could also try the following books published by the IChemE:

(1) Hazop and Hazan 4th Edn
(2) HAZOP: Guide to Best Practice
