HELLO!

I am working on a prestentaion regarding our flare system design.
What i wanted was to emphasize on correct design and importance of Flare in a refinery or petrochemical plant.
For this purpose i need information about industrial accidents caused due to flare failure or underdesigned flares etc with photographs.

your help would be highly appreciated.

Two relevant accidents come to my mind: Bhopal (1984) where toxic Methyl Isocyanate was relieved though a stack because flare was out of order, and Texas City Refinery (March 23, 2005) where a vent system had a too small KO drum.

I am sure you will easily find pictures, animations, movies etc. for these accidents e.g. by using Google or Wikipedia.
